Cannot see contact form

Hi everybody!

I add FORM section on page, add fields too. In CMS administracion i see added fields,
but in normal preview cant see!

Maybe FORM modul is damaged?
How to repair this?
Where is this module path? huh

WB does not offer a "preview" (unfortunately) ... what do you mean by that?

when i say preview, i mean view, normal view of page. Url address of page
