I used the "Form" module for create a form. I use WebSiteBaker Version 2.8.1 Revision 1287. How can broke a form in 2. In attach is 2 printscreen for what i have and what i need.

WebsiteBaker is not able to style the Form in this way. You could "rewrite" the output in the options of the Formpage and then style it with CSS to make it the way you want.

Or you use a WYSIWYG Page styled the way you want it and as action you submitt to the form module (haven't tried that though. Maybe one of the other formmodules is able to style the output like you want, but I haven't tested that myself.
