Form Module and Heading Problem

Creating a Form heading with the Form Module works great - shows my heading and then a line under it.  As soon as I reorder it up the page, the line disappears and the rest of my page goes screwy.  I thought it might be my template so I switched back to the default (I was using the expanded-menu Round, going back to Round) and that seemed to fix my problem, at least until I changed the header info - then I see the same problem.

Any ideas?
