Hi,

I can't seem to work out why when I add a form to a page is breaks the look of the website. The background doesn't load properly and the last div doesn't display correctly.

I have deleted the form and recreated it, but same problem. I must have changed something recently, can't remember what  huh, but I can't seem to get it to display correctly.

Do I need a separate index.php file, but only the basics in it and then point just this page to that amended index.php file? (Tried this and still had same problem).

You can see the form at http://www.pretoriahotel.com.au/pages/dining/functions-enquiry.php and if you click on another page you will see how it should display.

Any help would be appreciated, I am sure I am doing something simple wrong.

I see that your other pages use a wrapper such as #leftcolumn and #topwidecotent. Both of these IDs have background defined. You don't use a wrapper with your form that includes a background.
